Just By Being There: Nursing Beyond Limits’ In A Time Of Crisis

STANDING here—rather, trying to stand here because I just checked out of The Medical City for a bad back; forgive me if I seem a little intimidated before such a powerhouse group of health-care professionals. I refer to nurses, who are constantly in the front line of health care.

It is with nurses that patients are longest and most directly in contact in the patient’s most vulnerable state: administering drugs, taking blood pressure and temperature, monitoring the progress of our ailment and recovery, paying heed to our discomforts.

In mentioning powerhouse professionals, I refer also to doctors, whom I have observed the nurses sometimes consult.

What is evident is that it is nurses who are constantly observing the effects of the treatments prescribed, I guess to check if they are right, if they are working as expected, if they are too much or too little. And always and ever, attending to the patient’s constant need to feel less discomfort in this part or that, feel less pain. Standing as a constant assurance that whatever the hour someone who knows cares. The best nurses have this ability to make you feel like the only patient on the floor.

So naturally I am thrilled, although barely able to stand before those in whose hands, just a few days ago, I surrendered my fate: my fate as an invalid and my only hope of getting well, along with the doctors of course.

I am sure the doctors in the audience are chuckling and other members of the audience who are not nurses are amused.

Do not be. I am serious, for what I have just related is, if not the only, certainly the most prevalent and direct experience of medicine, medical attention and any other facet of health care that a patient has.

And it is the patient, after all, who stands, or rather lies in hospital gown, at center stage of the universal drama of human ailment and health care. Whenever the curtain rises on this drama, the spotlight is longest on the patient and his nurse.

Always has been, since Florence Nightingale

This was so since Florence Nightingale injected her presence into a man’s world and demanded more humanity from military surgeons on the battlefield, as they decided with cold detachment who was too hurt to go on living and who was worth the trouble of saving.

What of the rejected, and what of those whom the cure was killing more surely than the disease? It was for them that Dame Nightingale created modern health care and entrusted it to women.

From Dame Nightingale to Mother Teresa, the nurse’s code has been the hard and iron duty to be soft with those who are hurting, to be firm with those who are malingering, to watch at every step the progress of an illness or recovery so that nothing but wellness is irreversible; always and ever to care for as long as there was a living spark there. While there is life there is need for care, and if there is no more hope of recovery, there is always the imperative of nurturing every living moment that remains. Because life, however short it is expected to be, however long its suffering must extend, deserves a nurse’s total care. It is the patient, who may often be difficult and sometimes impossible to like because discomfort makes him so, who is the main focus of the nurse’s attention.

When the all-powerful French Cardinal Richelieu, the contrabida in The Three Musketeers movie, went to his dying king, it was not as a doctor to cure him but as a nurse to give him comfort, feeding him, from time to time, a teaspoon of raw egg. So it was not as a doctor that the great cardinal came to the king, to assure him that he would make him well as he had made him great, but as a nurse to comfort the king in his dying.

At this hour, I should be at my radio show, Karambola, laughing at all the problems of bad government. But this is a serious audience of people with serious commitments and concerns. And you are not in a laughing mood.

I don’t blame you.

The richest countries in the world lack nurses but no effort is made by them to make it easier to recruit our nurses, who are, far and away, the best. Instead, many nurses have to depend on unscrupulous recruiters here and worse employers abroad. The attitude abroad seems to be, let the nurses go through the eye of a needle and a gauntlet of overwork and underpay and other discriminatory practices, and then maybe, just maybe, let them stay on as second-class citizens.

Meanwhile, Philippine recruiters squeeze the nurses for more than they can afford before they leave, sometimes for nonexistent jobs abroad. Then they tie them down to financial servitude for the duration of their exile. Meanwhile the country’s foreign-exchange reserves grow from nurses’ remittances so politicians can change their ill-gotten gains to dollars for their junkets abroad.

Our country sorely needs more nurses; but going by the law of supply and demand and considering how poorly nurses are paid, you would think we had more than we know what to do with. I will let the private hospitals explain for themselves why this is so. But, coming from government, I can tell you the government has no excuse not to pay nurses more. The government is swimming in money. Look at the expected revenues, and look at the budget. There wouldn’t be a budget deficit if government were smart enough to spend more money on nurses than it throws away on bad projects for the kickbacks.

And, let me tell you, coming as I do from government, that the private sector could pay nurses much more if only the government stopped squeezing the private health sector for more and more taxes, and gave the sector as many tax exemptions, tax holidays and outright financial grants and easy credit as government does to so-called foreign and local pioneering enterprises. What is so pioneering about selling gasoline and yet government allowed Shell and Petron to escape taxes through a tax-credit scam that the government is too slow to prosecute up to now.

Poor national health care and the plight and proper place of the nursing profession in health-care reform, all these are problems to which the fastest and best solution is to throw money at them. Insufficient training of nurses after graduation? Subsidize their training with cash grants to the nurses and tax holidays to the training hospitals.

Ineffective nursing education? Throw more tax holidays and financial assistance at nursing schools; devote more funds to the stricter monitoring and regulation of teaching practices and educational standards in the nursing schools.

Is it wise to throw money at problems? Of course it is, as the most serious economists today recommend in the global crisis.

But if we do not have all that much money to throw around, we surely have more than enough to “devote to”—that is the proper phrase, “devote to” and not throw around—one of the highest priorities of government: the national health.

‘Beyond limits’

The theme of your convention—“Nursing Administrators: Leading Beyond Limits”—imposes on yourselves a part of the formidable challenge of national health care.

There’s no glossing over, no understating, the problems that our health care sector faces; much less the very real limits that tie the hands of those seeking to solve them. But since you have so bravely undertaken to go beyond those limits, I will try to walk with you down a road that’s hardly taken.

First of all, let me congratulate those of you who have opted to stay behind in this country, when we all know that the overwhelming majority of people studying to be nurses do so in hopes of landing better-paying jobs abroad.

I do not begrudge them for seeking not just greener pastures but pasturage of any description.

Nor do I conclude that those who chose to stay do not need the better pay that nurses get in First World countries.

Those who stay, attend to the sick over here. But they suffer the frustration of not being able to do as much as their training would allow them. Too few people can afford the quality health care that our nurses are equipped to give. And government isn’t spending enough to bridge the gap between increasing health care needs and the diminishing capacity to meet them—on the part of the Philippine health care systems, doctors, nurses, hospitals, clinics and on the part of patients.

Universal health care would create a whole new universe of nursing opportunities right here in our country—and government can afford it, if it only stops stealing.

Those who chose to leave, on the other hand, trade off family for better opportunity—many times so as to help family back home. What they get instead is years of loneliness and hardship, as they struggle to cope with a higher standard of living with what remains of their pay after the recruiters get their share.

These are tough choices.

Consider: a nursing graduate who lands a job in a small private hospital would be “lucky” to get a salary of P10,000 a month. The usual entry level is P8,000. Recently, as a result of the enactment of the amended salary standardization law, nurses in government hospitals are now entitled to a salary range of between P14,000 and P18,000; better but far below starting rates for Filipino nurses who make it to the US, the UK, Canada or, heaven forbid, the Middle East.

Little wonder that most nursing administrators are reeling from the fast turnover of newly trained nurses; no sooner have nursing administrators finished training one batch than another foreign-based recruiter poaches them.

Ironically, even as more and more nurses are being graduated, trained, and then recruited abroad, most hospitals—private and public—have the same complaint about not having enough qualified nurses. In short, we seem to have an oversupply of under-trained nurses. So we have too many “nurses” in quotes and too few nurses without quotes to go around. Many of the good ones are poached by foreigners.

Frustrating paradox

This paradox arises from a frustrating situation: the nursing employment “boom” in other countries fueled the rise of so many nursing schools in this country, including not a few diploma mills. This June, 32,000 aspiring nurses passed the board exams. Where will these go, when there aren’t enough hospitals that can give them adequate training?

Ideally, you shouldn’t shortcut the training of a nurse, and a young nursing graduate is expected to post many months and hundreds of man-hours in the wards, the emergency and trauma rooms, in ICU and surgery. But comparatively too few get the chance.

In hospitals where such training is taken seriously, there are even more frustrating trends. As soon as a nurse finishes the ideal period for training, the headhunters descend on the hospital to harvest them.

We see here a net resource transfer from poorer to richer countries. Long before nurses can be deployed abroad to earn enough to repay recruiters and remit their better wages to families back home, and thus make the brain drain worthwhile in other respects—long before that can happen, they are poached by foreign countries that did not spend a cent on their education and training.

I can commiserate with the exasperation of nursing service administrators who have struggled mightily, these past several years, to keep the critical balance of human resources in our health care sector, the relentless foreign poaching notwithstanding.

From this background, then, arise the most serious challenges facing the nursing service administration in the country:

First, the challenge of maintaining, always, a high quality of health care despite limitations in terms of physical facilities, financial resources, and the lack of adequately trained nursing staff given the quick turnover.

Second, the economic crisis that has made things worse for everyone all around; forcing budget cuts in state hospitals and cost cutting in private ones, and setting further back hopes of salary upgrades for nurses. Worse, costly and too strict medical insurance forces most people with serious ailments to forego treatment, thus adding to the difficulty of caring for them when they are too far gone.

Third, the mismatch between supply and demand, not just in the number of nurses, but also in the kind of job and career opportunities that are opening up to our nursing graduates. Even as thousands of them are needed in the lesser cities outside the great metropolitan centers and worse yet in far flung areas that have never been reached by any decent health care program, there aren’t enough nurses to go around.

I am not suggesting that nurses should sacrifice personal income for outreach services. I am saying that the national government, which throws tens of billions at sure-to-fail, never-intended-to-succeed projects, just for the commissions, could throw more money at making health care reach more people, even to the farthest corners of our country.

On the third challenge, most of you are aware of how medical tourism, which the government is understandably encouraging, has drawn away many of our best nurses from serious health care to spa services pretending to offer real medical solutions to insoluble old age and wear-and-tear problems like cellulite, wrinkles and osteoporosis.

Don’t get me wrong, but it seems to me we can’t tolerate for long a situation where thousands of our nursing graduates are doing work as call-center agents or spa attendants or running small businesses because there aren’t enough good hospitals paying decent wages.

You should know that no country in the world could support a well-functioning public or private hospital system—or both—without universal health care coverage. In short, without government paying for most health-care services.

Pardon me if I seem to be in a griping mood. There just seems to be little in the local health-care sector we can see as a bright spot.

Some reforms

To be sure, we have seen the start of the implementation of a landmark law, The Cheaper Medicine Act. That reform didn’t come easy. The foreign drug firms were all over Congress. I objected to their presence. They said they had the right to be there because the offices of European and American drug companies are in Makati. They tried to “influence” me in quotes. I said I did not remember giving them the right and threw them out of Congress. It took all of 10 years to get the law passed and then another year to get it implemented.

Reforms in health care come hard but not, I dare say, slowly. Not if you have the political will and the sincere desire to help.

It didn’t really take 10 years to pass The Cheaper Medicine Act. There were many attempts over 10 years but only one night of fighting to get it passed. It didn’t take almost a year to get it implemented but just one morning of threatening the Department of Health to act.

So don’t believe anyone who tells you it takes time to make reforms. It takes one day, one will, hell, one person even—to lead the pack and win the day.

You want pay commensurate with service? Then join those of us who are telling government to stop spending on itself and start spending on the people—especially on people who make a difference such as health-care providers like nurses.

And doctors, of course; we can see how useful they are to nurses, such as in instructing them on what medication to give and in what dosages, as nurses go about the time-consuming, all absorbing task of direct, unceasing, person-to-person health care.

But even as you continue to push the envelope on upgrading nursing performance through rigorous and focused strictly health care training, I trust you’ll always remember that besides those skills, our people look to the nurse as the one special person who fills in all the gaps of a health-care system—whether it’s for a doctor who’s too busy to check on the patient as often as needed; a malfunctioning equipment for which they must improvise; or an ill-tempered billing clerk; or dirty facilities and dirty linen or lack of clean water in the hospital.

It is effective nursing administration that ensures a steady supply of the kind of nurse who, although really powerless to do everything the patient wants or needs, seems to make it all better just by being there.

And that, I think, is the core meaning of “leading beyond limits.” For beyond the limitations imposed by limited resources, limited manpower, limited this and limited that, is someone who fills the empty places by something intangible but evidently there: a constant caring for the patient who is, in every room if she can afford it, or in every space in a ward if he cannot, always at center stage in the unceasing drama of health care.

I return to where I started…to the nurse as she or he should be: competent, for sure, and always caring; gentle yet firm; seeing her profession as a vocation and not just a job. That is why a Filipino nurse or, for that matter, a Filipino doctor, working abroad are still a nurse and a doctor and not economic refugees.

It is the highest vocation of all, the equal of priest or religious, though I am told nursing is much more fun: the cure of bodies to the cure of souls.

RP chosen for new healthcare KPO hub

MANILA - First, there was business process outsourcing for college students. Now, graduating Filipino nursing graduates who are looking for work and a chance to increase their skills further can do so by joining the knowledge process outsourcing (KPO) arm of Life-Stage Communications (LSC) Fly Free For Health.

Geoff Spiller, LSC joint managing director, said the LSC-KPO arm functions the same way as a call center but with a more specific focus on the heath needs of their clients. He said the company is looking to hire at least 100 Filipino health professionals in the next six months who will work as "medical butlers" and provide borderless healthcare to clients.

Spiller said the Philippines has several advantages not found elsewhere that makes it perfect as a KPO hub for the company.

"The Philippines has call center expertise that doesn’t exist elsewhere. It's got ample human resources who have good English language skills. Finally, there are so many nurses who are looking for jobs," he told abs-cbnNEWS.com during the company's launch on Wednesday.

Spiller said the KPO medical butlers are highly-skilled individuals who all have medical backgrounds. "They are going to be talking to customers who will ask them all sorts of questions. You have to have the basis to give information and refer the customer to the right person," he said.

Dr Wei Siang Yu, founder of Fly Free For Health, said the convergence of technology and global value chain management has made the evolution of healthcare business process outsourcing possible. He said the company's value chain of services, including medical concierge management, borderless multi-disciplinary management and borderless collaborative care, could help drive new patient leads, loyalty and repeat transactions for medical facilities.

He said the company also specializes in development of international medical tourism destinations, particularly in Southeast Asia and countries such as Korea, India, and Australia.

Spiller said the company is looking for an office in Pasay City for its first KPO center. He said the company is partnering with RA Gapuz Review Center, the largest review center for nurses in the Philippines, to offer KPO courses to willing students.

“These introductory courses are going to be a revolutionary step for nurses who are seeking alternative paths to nursing practice. We are confident that nurses in the Philippines are going to be very competitive in the global market of borderless healthcare,” said RAGRC president Dr. Mia Gapuz.

At long last, the June 2009 Nursing Board Exam Results (Nursing Licensure Exam) is released!

A total of 32,617 out of 77,901 passed the Nurse Licensure Examination, the Professional Regulation announced Saturday.

Golda Manto Yap, a graduate of the Felipe R. Verallo Memorial Foundation, topped the exams, getting an average score of 86.80 percent.

The Board of Nursing conducted the test last June in the cities of Manila, Baguio, Cagayan de Oro, Cebu, Davao, Iloilo, Legazpi, Lucena, Tacloban, Tuguegarao, Sulu, Pagadian and Zamboanga.

COPAR Community Assembly

Yep! That's me on our COPAR Community Assembly. I'm one of the speakers that time and I was interpreting and explaining the data we have gathered from our community survey. My topic was about the community's immunization status, their drug habits and the most common diseases found in the community.

While the talk is going on, some of my classmates are also busy taking blood pressure at the back for those who want to kno their blood pressure.

We also prepared snacks for the 50+ people who attended our Community Assembly and some raffles for those who can answer the questions we throw at them. Of course, the questions are related to the topics discussed, so they really have a reason to listen to our discussions!

The COPAR Community Assembly is the last and most tiring part of our COPAR Exposure so we took some pics with all the members and Clinical Instructors present.

I'm really going to miss this experience and especially my time with my classmates.

Feeding Program in the Community | COPAR

After our Community Health Teaching and our Operation Blood Pressure, another activity we had while on our COPAR experience is the Feeding Program. As you can see, I'm already in some of the photos so I'm inspired to blog about this one! This is one of the most fun experience we had in our COPAR because we get to mingle with a lot of kids and not only that, we have made them happy by our Feeding Program. This was held in the Sitio Nazareth Day Care Center so a lot of kids were really there waiting for their classes to start.

We have taken a lot of pictures so I don't need to narrate everything that happened because a single photo is worth 1000 words. And I don't think I have that 1000 words so let the pictures narrate the story. ^_^

According to the ABS-CBN news last March 4, 2009, thousands of Filipina Muslim and Non-muslim nurses are in demand in Saudi Arabia.

Actually, there are already 2000 Job offers for filipina muslims in POEA, but only 600 have been filled yet so there are still 1400 vacant job offers. Since filipina nurses are in demand in other parts of the world, Saudi Arabia decided that they would accept filipina nurses even though they haven't taken or passed the Nursing Board Exam yet as long as they are muslims.

Not only muslim nurses are in demand in Saudi Arabia. They still have 5000 Job vacancies for filipina muslims and non-muslims.

According to DOLE, a nurse in Saudi Arabia can receive 2250 to 4000 Saudi Arabian Riyal or more or less 50,000 Philippines Peso. Aside from free housing, wages are also tax-free.

Our 1st Day Of Community Organizing Participatory Action Research (COPAR)

Before I talk about my experience on our first day of COPAR, you may want to check out first what COPAR Nursing really means.

Our first day of COPAR happened last Jan. 20, 2009. Yeah I know, this post is already too late, but don't care anyway. Hehe! I just have to post this one because it's one of my greatest experience in COPAR.

Our group was assigned in Barangay Buhisan, Sitio Nazareth, Cebu City for our COPAR Nursing. Before we arrived at the place, our Clinical Instructor already told us what to expect in there. he said that the place was settled in a Valley, and you can just see Mountains around you where most houses are settled. And he was right! We still actually have to climb mountains just to interview a family for our Family Health Assessment form! But this actually, is what makes COPAR so fun and exciting.

The first picture above was taken at the peak of the mountain we climbed on our first day. Very scenic right? But before you can reach that, you have to go through a series of "phases" first!

1st Phase: The Ascending Phase

That's steep you know! You may think it's easy but it's not! This is where we can test our BALANCE It's very easy to slip because of our shoes and because of the rocky road. Actually there were some who slipped that puts us to the next phase!

2nd Phase: The Sobbing Phase

See? I told you it's not that easy! Imagine if one falls, then all the other people behind might also fall! This phase is where we can test our FEARS and EMOTIONS.

3rd Phase: The Vanity Phase

Now that were at the top, all the fears, the anxieties and the sobbing are gone. But then, vanity strikes again! tsk3! I wonder what test this phase gave to us?? ?_?

4th Phase: The Descending Stage

Actually, climbing down the mountain is much harder than climbing up! This is where we can test the STRENGTH of our leg muscles!

Emphasis of COPAR:

Phases of COPAR Process:

1. Pre-Entry Phase - is the intial phase of the organizing process where the community organizer looks for communities to serve and help. Acitivities include:

Preparation of the Institution

Train faculty and students in COPAR.
Formulate plans for institutionalizing COPAR.
Revise/enrich curriculum and immersion program.
Coordinate participants of other departments.

Site Selection

Initial networking with local government.
Conduct preliminary special investigation.
Make long/short list of potential communities.
Do ocular survey of listed communities.

Criteria for Initial Site Selection

Must have a population of 100-200 families.
Economically depressed.
No strong resistance from the community.
No serious peace and order problem.
No similar group or organization holding the same program.

Identifying Potential Municipalities

Make long/short list.

Identifying Potential Barangay

Do the same process as in selecting municipality.
Consult key informants and residents.
Coordinate with local government and NGOs for future activities.

Choosing Final Barangay

Conduct informal interviews with community residents and key informants.
Determine the need of the program in the community.
Take note of political development.
Develop community profiles for secondary data.
Develop survey tools.
Pay courtesy call to community leaders.
Choose foster families based on guidelines.

Identifying Host Family

House is strategically located in the community.
Should not belong to the rich segment.
Respected by both formal and informal leaders.
Neighbors are not hesitant to enter the house.
No member of the host family should be moving out in the community.

2. Entry Phase - sometimes called the social preparation phase. Is crucial in determining which strategies for organizing would suit the chosen community. Success of the activities depend on how much the community organizers has integrated with the commuity.

Guidelines for Entry

Recognize the role of local authorities by paying them visits to inform their presence and activities.
Her appearance, speech, behavior and lifestyle should be in keeping with those of the community residents without disregard of their being role model.
Avoid raising the consciousness of the community residents; adopt a low-key profile.

Activities in the Entry Phase

Integration - establishing rapport with the people in continuing effort to imbibe community life.

- lend a hand in household chores
- avoid gambling and drinking

3. Organization-building Phase

Entails the formation of more formal structure and the inclusion of more formal procedure of planning, implementing, and evaluating community-wise activities. It is at this phase where the organized leaders or groups are being given training (formal, informal, OJT) to develop their style in managing their own concerns/programs.

a graduate of Baguio Central University,
is the Nursing Board Exam topnotcher, with a rating of 89%.

MANILA, Philippines – Despite the world financial crisis that has affected even employment opportunities for nurses here and abroad, Nursing Board topnotcher Jovie Ann Alawas Decoyna, has remained optimistic and urged her peers to take the initiative in uplifting the profession.

“We should take the initiative to look for the job rather than being idle. It is our responsibility to uplift nursing as a field,” said Decoyna, a graduate of Baguio Central University, who was one of over 88,000 hopefuls that took the Nursing Licensure Exams in November 2008.

A total of 39, 455 passed the exams, the results of which were released by the Professional Regulatory Commission this Friday.

Decoyna said that while the proposal by the Commission on Higher Education to extend the present four-year nursing curriculum to five years would burden parents, she admitted that it would nonetheless help improve the curriculum.

“It should improve the clinical practice of Nursing and mold nursing students into competent nurses,” Decoyna told INQUIRER.net in a phone interview.

Nursing is Decoyna’s second degree. Its caring aspect motivated her to pursue the course, she said.

BS Biology was Decoyna’s first degree, which she earned at Saint Louis University in 2005.

“I see Nursing as an opportunity to help my family. I owe everything to my family and friends who supported me along the way,” answered Decoyna when asked why she took another degree after Biology.

Decoyna said that she was open to the idea of working abroad but, given the opportunity, would prefer to work in a hospital in Baguio.

To her fellow Nursing board passers, Decoyna has this to say, “Dapat maging honest tayo at tulungan ang sarili na maging competent at compassionate [We should be honest and help ourselves to be competent and compassionate].”

At long last, I was already able to meet the CBS or Cebu Bloggers Society! I submitted my membership form first or second week of January I think and because of the preparations they made for the Sinulog 2009, they were not able to process my membership to CBS.

Luckily, this week, the membership coordinator of CBS, Evanjohnn Mendoza texted me If I was available for meet-ups, being one of the requirement to be a CBS member. I have no classes on Saturdays so I decided to schedule it at 3 PM today because my other schedules were already taken(ehem.. feeling hectic). At 8AM-12PM, I had to go to the practice for the cotillion for my friend's debut, and at 7PM, I had another business meeting with another guy. So the best time for me was at 3PM.

The funny thing was that, while I was already riding a jeepney on my way to Ayala Mall, I noticed some guy ON the jeepney bringing a laptop. At that moment, I jokingly thought that "What if he's the one who's going to meet with me??". And at my second glance at him, I noticed his shirt. I was only able to read the letters "GGER" and that time, I was like, "OMG! I have to read the entire text on his shirt!" My heart was poundind at that time while I was patiently waiting for him to "move" so I can read the entire text. And when he slightly moved, I was able to read it, "Blogger ko Bai!" and below it read "Cebu Bloggers Society". What I did?? I turned away from him and laughed at myself! I continued laughing until the jeepney arrived at Ayala Mall. But to make sure that he's really the one, I just followed him and see where it leads me. Our meeting place was at Bo's Coffee in Ayala and while I was following him, I noticed that he is on the right way to our venue. He was also in a hurry so I thought, "Oh, maybe he really is the one because he's hurrying up for the scheduled meeting!" So to cut the story short, I decided to talk to him even before we reached our venue and found out that he really is the one who texted me to meet up with him. None other than Evanjohnn Mendoza himself. He was nice and I was glad to finally meet him.

And lucky me, the CBS also had a meeting at 3PM so it was really an opportunity for me to meet other Cebuano Bloggers not just the membership coordinator. When we arrived at Bo's Coffee, another blogger, Sinjin, who I happen to know at Multiply.com was already waiting outside. We were the first three to arrive and we were able to talk a lot to each other. Then one by one, other Cebuano Bloggers came and I was excited to talk to them. They were all good and experienced bloggers. This is the list of those who came:

And there was another girl who arrived when we were about to leave. I never got her name and blog. All I knew was that she was from Cebu Doctors University. The Philippine Nursing Board Exam Results for November 2008 is almost here! It has been said that the Nursing Board Exam Results for November 2008 will be released sometime in February. The November 2008 Nursing Board Examination or is otherwise known as Nursing Licensure Examination has the highest number of examinees with almost 90,000 examinees. The November 2008 Nursing Board Examination has the highest number of examinees because this is the time that ALL schools that are offering Nursing Courses for the first time or Pioneering Schools for Nursing graduated. One example here in Cebu are the students from the University of San Jose Recoletos. One of which is my sister's Ex-boyfriend! I just wish he would pass this exam. ^_^

Can Blood Type Determine Your Personality?

I have a rare blood type—AB+, which only 4 percent of the U.S. has—and secretly, I always thought it made me kind of special, even though I had no basis for that belief. But in Japan, blood type is used to determine one’s personality and temperament. Like astrology, the scientific community often dismisses blood type as an indicator of personality, but that doesn’t stop its popularity in Japanese women’s magazines, where blood type horoscopes are published. In Japanese video games, blood type is commonly a dimension in character creation. In fact, some Japanese might even be shocked if a foreigner didn’t know her blood type.

The history of the theory that blood type determines personality is actually a dark one. In 1900, Austrian scientist Karl Landsteiner discovered the four blood types. Like 19th-century scientists who used brain size (the larger the brain, the more advanced) to propagate theories of racial and gender superiority, many (white) scientists used the blood type theory as a way to push forward their own ideas of racial supremacy. Scientists from the west suggested (falsely, obviously) that Asians were inferior and sub-human since the vast majority of them—like animals—had type B blood. In 1927, Takeji Furukawa, a Japanese professor, introduced the theory to the Japanese public in a paper entitled, “The Study of Temperament Through Blood Type.” Even though he lacked the proper credentials and backed up nothing scientifically, the public embraced the idea of blood type as a determination of personality. The Japanese government even commissioned a study to determine if they could breed better soldiers. In the 1970s, a book by Masahiko Nomi revived the idea, which is still popular with the general Japanese public. In 2008, four books on the topic have hit Japan’s top ten bestseller lists.

Find Out Your Blood Type
One of the easiest ways to find out your blood type is to give blood. Since it has to be matched for donation, they type it anyway, so you can always ask. If you’re pregnant, doctors administer certain blood tests to assess potential genetic problems, so determining the mother’s blood type is standard. You can ask your doctor to perform a test when you get blood drawn, but since it isn’t usually medically relevant, you may have to pay for the test. The easiest way? Ask your parents; they probably know.

What Your Blood Type Says About Your Personality

Type O
You are the social butterflies. Often popular and self-confident, you are very creative and always seem to be the center of attention. You make a good impression on people and you’re often quite attractive. Organized and determined, your stubbornness will help you reach your goals. You make good leaders. Lovewise, O is most compatible with O and AB. Common career choices: banker, politician, gambler, minister, investment broker, and pro athlete.

Type A
Type As may seem calm on the outside, but inside, you’re filled with anxiety and worry. You’re perfectionists and often shy and sensitive. Usually introverted, you’re stable and thoughtful. You make good listeners and are sensitive to color and your surroundings. You like to be fashionable and are up on the latest trends, but never flashy or gaudy. You like romantic settings and often shun reality for fantasy worlds. A is most compatible with A and AB in the love department. Common career choices: accountant, librarian, economist, writer, computer programmer, and gossip columnist.

Type B
You can be very goal-oriented and often complete the ambitious tasks set before you. Outgoing and very charming, you’re good at reading people and providing support. Though critical of appearance (but not your own), you aren’t picky and are unlikely to dwell over the little things. Type Bs are impulsive individualists who often create their own path in life. You are very strong and optimistic. B is most compatible with B and AB lovers. Common career choices: cook, hairdresser, military leader, talk show host, and journalist.

Type AB
Not surprisingly, ABs can be quite dualistic, possessing both A and B traits. You may be shy and outgoing, and hesitant and confident. You often stand out from others, don’t like labels, and are nice and easy going. You are logical and determined to do things correctly. Usually trustworthy, you like to help others. You often speak in a serious manner. Your patience, concentration, and intelligence are admirable. AB can find a soul mate with any other blood type. Common career choices: bartender, lawyer, teacher, sales representative, and social worker.

We all love to know things about ourselves and blood type is just another fun way to learn new traits and understand why we do certain things. You don’t have to believe in it to get something out of it—just have fun with it and take it with a grain of salt. My blood type is actually considered the worst in Japan; no one wants to work with ABs and most anime villains are that type. Thank God I live in America!

Community Health Nursing: What's so good about it??

I should have posted this topic long ago but I always forgot until it finally went out of my mind. It just came back to me when we started discussing again about Community Health Nursing in our lecture today. So, I would like to share some of my experiences on our CHN duty or Community Health Nursing and what's so good about it.

I'm used to hearing my sister say that having a duty in the hospital is better than in the community. She said that it's very tiring when in the community. But when it was my turn to have a duty in the community, I proved her WRONG!! ^^ My experience in the community was great. We had great time with my RLE group. And do you want to know what made it so great??

The first three photos were taken in the house of one of the families assigned to one of my classmate to conduct a study for our Family Heal Survey and Family Nursing Care Plan. His family was so nice. His family cooks food and sells it in their community, and we "the guys" were the luck ones to taste it! And I tell you, It tastes great! The food was a "tinola" or "souped-fish" It really was a great time for us! The fourth photo (I hate this photo, I'm nowhere to be found because I was the one taking the picture!!) was taken in a bakery just in front of the Barangay Health Center!

It's just not the food that makes it great. When pre-natal day comes, Dozens of mother's with their babies come and our task that time is to take their vital signs. I volunteered myself to be the one taking the mother's blood pressure and the babies temperateure rate. It was cool because there's just so many mothers and I get to practice my blood pressure taking skills, and in the shortest period of time, to cater all of the mothers. We also had our flower arrangements while in the community.

And after the fun day, we again took pictures, this time, with me, because the one taking the picture was already the Barangay Health Worker.

Worst Science Article of the Week: Drinking Coffee Shrinks Your Breasts?

If you have read my post about Too much coffee can decrease your breast size?? Then you maybe already very worried now, but don't panic! Thanks to some fellow blogger (who criticized my post actually, hehe!), gave me some links to a new findings. It is said that it's not true that Drinking coffee could shrink your breast size and that coffee and breast size have no relation. Just read the whole post:

Female coffee drinkers beware: that Pumpkin Spice Latte might shrink your breasts. Or so you would think, if you scanned the headlines last week. A new study in the British Journal of Cancer [subscription required] has incited mass hysteria over a tenuous link between coffee intake and breast size. The Telegraph warns: “Drinking Too Much Coffee Could Shrink Women’s Breasts,” while UPI throws in a pun: “Study: Cups of Java Cut Cup Size.” But the best comes from the New York Post: “Women Face Drink & Shrink Dilemma, Coffee Poses a Booby Trap.”

But before you pour that cup of coffee down the sink (or “accidentally” spill it on your busty archnemesis) let’s take a closer look at that study:

Researchers from Sweden recruited 269 women (average age was 29) to have their breast size measured and to answer a questionnaire about coffee intake and other lifestyle choices. All the women were from families at high risk for breast cancer and about half carried a gene, CYP1A2*1F, that is associated with breast cancer. Essentially, the researchers were studying the relationship between CYP1A2*1F, breast size, and coffee intake. The gene is known to control the metabolism of the hormone estrogen as well as certain chemicals found in coffee; it’s also been linked to higher breast density and thus higher breast cancer risk.

This is what they found:

For women who had CYP1A2*1F, those who drank 3 or more cups of coffee a day had smaller breasts than those who drank less coffee (average difference was about 150 mL). However, for women who did not have the gene, drinking more than 3 cups of coffee was actually associated with slightly larger breast size (average difference was about 50 mL). The researchers do not have an explanation for the correlation between coffee and breast size. They only conclude that because coffee was correlated with smaller breast size, it supports earlier studies that suggest coffee protects against breast cancer.

But just about all the media accounts gloss over the genetic element and focus on cleavage shrinkage. Much of the problem lies in the use of the word “shrink” (which appears nowhere in the paper itself) to suggest that breasts will actually inflate and deflate based on coffee intake. As the NSF points out, this was a cross-sectional study that took data from one point in time and “because such a study doesn’t look at time trends, it can’t establish what causes what.”

Though the researchers controlled for some factors such as smoking and BMI, there are many others that could’ve affected the results. It’s likely that a woman who drinks 3+ cups of coffee a day would have a different lifestyle than a woman who doesn’t drink coffee. The researchers also point out that breast volume is not the same as breast density, but they chose to measure breast volume because it was easier. And how did they measure breast volume? They approximated the breasts as pyramids using the equation: base area x height/3.

So unless you believe your breasts are pyramid-shaped, don’t worry too much about the coffee-induced “booby trap.”

Too much coffee can decrease your breast size??

I know that most nursing students, stay late up night especially during examinations to study. I do that too. And the number one remedy to control the dizziness is to drink coffee right?? But how many cups do we usually drink? I know some of my schoolmates can consume 3 glasses just for that period of time! And most of it are girls! But do you know that drinking too much coffee can SHRINK your BREAST SIZE?? This is not only for nursing students but to all the women out there who drink a lot of coffee! I just want you to be aware of the fact(or so). hehe! Well, be the judge, just read this article I have found.

LONDON: Ladies, please note -- drinking coffee in moderation may be okay, but downing more than three cups daily can shrink the size of your breasts, a new study has revealed.

Researchers in Sweden have carried out the study and claimed that women who take more than three cups of the caffeine-fuelled drink a day could see their bra size drop, the 'Daily Star' reported.

Tests by cancer researchers found half of all women have a gene linking breast size to coffee intake.

Nearly 300 women were quizzed but Helena Jernstroem, of Lund University, said women should not worry too much.

She explained: "Coffee-drinking women do not have to worry their breasts will shrink to nothing overnight. They will get smaller, but the breasts aren't just going to disappear.

"Anyone who thinks they can tell which women are coffee drinkers just from their bra measurements will be disappointed. There are two measurements for a bra -- the cup size and the girth, so you wouldn't be able to tell."

The study showed regular hits of caffeine reduce the risk of women developing breast cancer.

HOW IT FELT WHEN I WAS "DEAD"

Yeah.. It really is me.. "dead". And anyone who reads this post, even this first line, will be HAUNTED by me for the rest of their lives! BWAHAHA!

JUST KIDDING GUYS! I can't do that to you, I love all my readers! hehe! But really, the guy in the photo is really me.. no joke. I'll show you another proof. ^_^

There you go! It really is me! ^_^ That happened when we had out Return Demonstration for Post-mortem care. It was kinda fun to play dead you know. But I really didn't have fun doing the Post-mortem care to my partner! I missed a lot of procedures and even forgot to bring some of the materials needed! Grrr! Im sure I got the lowest score on this demonstration. But I don't care anyway. ^_^ Playing dead was a lot more fun.. you get tied on the hands, you get tied on your head, and feet tied too. You even get to experience wearing an adult diaper too! (See in the picture) But all the fun ends there! After that, you're going to be wrapped in a blanket. I mean "wrapped" as in "Lumpiang Shanghai"! No body part should be exposed inculding the head and you'll remain wrapped until your partner is done with the procedure! And with no air-conditioned rooms, you'll feel like you're in an oven, ready to me heated! You'll even have a hard time breathing. So if you think being dead was fun?? Not a chance! You'll die trying to breath and trying to cool off your body! Haha!

POST-MORTEM CARE: ROLE OF THE NURSE

I. Purpose:
To clean the patient's body and prepare it for removal from the hospital after death.

II. Responsibilities of the Nurse

Care of the Valuables

a) Valuable with the patient at the time of death should be identified, accounted for and sent to the proper department of the institution for safe keeping until family claims for it.
b) Valuables taken or given to the patient's family should be voted on the form sheet specified by the institution.

2. Care of the body

a) Post mortem care of the body is not rendered unless the physician has pronounced the patient to be dead
b) Indicate the chart time the patient was pronounced dead and the physician pronouncing
c) Have the body cleaned and properly identified
d) If the patient is communicable the water used in cleaning must be medicated
e) Inform medico legal Officer if accident, suicide, homicide, or illegal therapeutic practices causes death
f) Prevent all means of distortion, discoloration or scarring of the body as it is distressed to the family.

3. Accomplishment of necessary papers related to death. Death certificate is sent to the local health Department. It is accomplished by the physician and signed by him and by pathologist if autopsy has been performed.

4. Attitude towards the family. Provide all means to comfort the family and relatives who may be around.

OUR VERY FIRST DAY OF EXPOSURE IN THE HOSPITAL

GUYS! I apologize for the three consecutive posts about performance checklist. Many students are just begging me to post some of my notes. And I also want this blog to be very informative and very useful to students who needs it. Please bear with the posts. ^_^ I'll make sure to post more on our experiences. ^_^

So here's the footage from our first day in the hospital. Our group was assigned on the Out Patient Department and we were very much lucky with it! The Out patient department was the least stressful from all of the wards! Unlike in other wards, we were only assigned to guide patients to their respective rooms and take their TPRBP or Temperature, Pulse rate, Respiratory rate, and blood pressure. And most importantly, we were required to do DUSTING every morning!!! GRRR! But at least our dusting skills had improve! haha! ^_^ But still, our group was the most relaxed in our section, the BAD thing is, we just can't relate to our other clssmates talking about NGT and IV insertion because theirs no such things as that on the out patient department! T_T But anyway, we'll still get to that part. ^_^ So if your assigned in the OPD for the first time, all I can say is, ENJOY and RELAX! ^_^

BED SHAMPOO: PERFORMANCE CHECK LIST

Assist patient to recumbent position with the kelly pad under the patient's shoulders
Brush the hair thoroughly
Place one cotton ball to each ear.
Place the third towel over the chest
Place damp wash cloth over eyes
Test the temperature of the water
Wet the hair and apply shampoo. Rinse well
Wet the hair again and apply shampoo. Rinse well
Apply the lemon juice. Rinse with water
Remove the kelly pad or rolled rubber sheet and place it in the pail
Return pillo to its proper postion under the head
Remove earplugs and damp wash cloth.
Dry and comb the hair

TEMPERATURE, PULSE RATE AND RESPIRATORY RATE: PERFORMANCE CHECK LIST

Temperature - is the degree of heat maintained by the body as measured by a predetermined scale. It is the balance between the heat production and heat loss of the body
Pulse - the rhythmic expansion of an artery produced by increased volume of blood forced into by contraction of the left ventricle at each heart beat
Respiration - the exhange of gases between an organism and its environment

II. Preparations

Blood Pressure - the force exerted by the blood against the walls of the blood vessels as it flows through them.

II. Preparation
A tray with the lining containing the following:

Sphygmomanometer
Stethoscope
Cotton balls soaked in alcohol
Wash cloth or hand towel
Waste container
Tissue paper to line the BP cuff

III. Performance check list

Wash hands and gather equipment
Wipe the earpieces of the stethoscope with cotton balls
Explain the procedure to the patient
Apply blood pressure cuff on arm
Locate patient's brachial artery.
Place earpieces of stethoscope into the ears
Close valve on hand bulb.
Place diaphragm of the stethoscope over the brachial artery

Wipe earpieces with cotton balls soaked in alcohol
Record appropriately, including narrative if necessary.
Return the apparatus to its proper place.

WHAT BRAIN ARE YOU?

I saw this video from The Secret of Life blog and I thought that this video might help students, especially nursing students to survive our course. This video will tell you whether you're using your right brain or your left brain or both!! I'm just having some doubts if this really is the real thing or just one Optical Illusion.

It state's that:

If you see this lady turning clockwise, you are using your right brain. If you see it the other way, you are using your left brain. Some people do see both ways, but most people see it only one way.

If you try to see it the other way and you do see, your IQ is above 160, which is almost a genius. Then see if you can make her go one way and then the other by shifting the brain's current.

Both directions can be seen! This was proved at Yale University over a 5 year study on the human brain and its functions. Only 14% of the US population can see her move both ways.

At first try, I was able to see the girl turning clockwise. Which means I'm a right brainer and they say that Right-brained subjects, focus on aesthetics, feeling, and creativity. While left brain subjects focus on logical thinking, analysis, and accuracy. In general, schools tend to favor left-brain modes of thinking, while downplaying the right-brain ones.

Now that explains why I don't like instructions given by the school because I want to do it my way. And I know I can finish my tasks efficiently if I do it my way and not the school's way! ^_^

But when I tried to focus hard, I was able to see it the other way! Which means my IQ is above 160! Yipee! Good news for me! ^_^ How bout you? Try to find out! ^_^

Yeah! At long last! I was able to open my blogger account! It's been a while since I last posted something on my blog! I missed blogging sooooo much! And I'm also very happy that ALL our requirements in school is already done! And only 3 more days to go.. It's already our final exams! Then.... semestral break! Yipee! I'll be able to blog all day and all night! Can't wait!!! ^_^

Hmmm.. You might be wondering now why the title of this posts is "Student Nurses are also Information Technology Students". Actually, one of our requirements for our final exams in our Information and Communications Technology Subject (or is otherwise known as "Computer Subject") is to create two videos. One is a music video and the other one is a documentation film. My group is required to make an OPM (or Original Pinoy Music) music video and our chosen song was Magbalik by Callalily. The setting of our video is just in our school and the story is about a Guy who was caught by his girlfriend dating with a "gay" guy! His girlfriend dumped him and because of his depression, he ate too much "side-street" food. Unfortunately, Karma maybe, the guy got LBM (Loose Bowel Movement) and always going back to the CR. Magbalik in english simply means "to return" which is also our point in the movie, to Return to the CR! ^_^ Fortunately, he bumped on a Girl (who was also eating the "side-street" food) in the CR!

And since I was not able to blog for a long time, this is my treat for you guys! This is the fruit of our labor, from our tiring, stressful but fun week! Hope you enjoy the movie! And for those who want to download the movie.. I'm currently uploading it right now. I'll put a link for download after the upload is done. Thanks! Enjoy our movie! I admit that bathing is a very refreshing, stimulating, thirst-quenching thing to do. But this ones entirely different! We all had the chance to be the patient(the one to be bathed), and I thought at first that this is gonna be great! A break from our stressful days. But it just turned out to be a more stressful day for us! Instead of having that, refreshing, stimulating, thirst-quenching feeling, we all ended up with head aches and an icky feeling! Unless for patients who haven't bathed for years of course! haha! But one things for sure, I WON'T BE BED BATHED AGAIN! NO MORE! ^_^

ROUTINE DAILY CARE FOR THE PATIENT: BED BATH

Purpose:

To clean the skin thus eliminate waste products
To stimulate circulation through slightly active or entirely passive exercises
To refresh the patient by relieving fatigue and discomfort

I. Preparation
Equipments: A tray containing the following:

Basin with warm water of 105-110 degrees Fahrenheit
Two (2) bath towels
Two (2) wash cloth
Drinking glass
Toilet articles
Hair brush
Comb
Tooth brush and tooth paste
Scissors
Powder
Toilet Soap
Deodorant
Bath blanket
Two (2) pitchers
Bath thermometer

II. Procedure

Wash your hands (as always.. hmm..)
Screen the bed
Replace top sheet with bath blanket
Remove gown
Put one towel under patient's head and one on the chest
Test temperature of water
Dip washcloth in warm water. Sponge face, ears, neck. Rinse and dry.
Place bath towel under arm and shoulder from you. Sponge axilla. Rinse and dry.
Soak hands in basin of water. Soap and rinse.
Drape chest and abdomen with bath towel. Sponge, rinse and dry.
Turn patient towards you. Cover back after sponge. Massage. Use firm long strokes.
Place towel under leg. Drape well. Sponge, soap, rinse and dry.
"FINISH THE BATH" (you may be asking what "finish the bath means", i'll post it next.^_^)
Apply deodorant. Remake bed. Comb patient's hair, clean nails.
Leave until unit is neat, order and clean.

I'll post some pictures too.. I have no more time now. Gotta go to sleep! hehe! Hope this can help! ^_^

I. Definition

Bed making - is the art of arranging linens
Closed Bed - is used to designate the hospital bed which remains empty after admission of another patient.
Open bed - used to designate the hospital bed when it is occupied or about to be occupied by a patient.
Ether bed or anesthetic bed - is one which it is made ready to receive a patient who has undergone anesthesia.

II. Preparation

2 white sheets
1 draw sheet
1 rubber sheet
1 bath towel
1 hand towel
1 face towel
1 pillow
1 pillow case
1 bedspread
1 blanket

III. Procedure

CLOSED BED

Arrange the linens in order as they are needed
Bring the preparation to the bedside of the patient
Take the bottom sheet; spread it on the mattress, placing the smaller hem at the foot part. Place it lengthwise on the middle of the mattress with the side equidistant from the center
Tuck the sheet under the head of the mattress. Miter the corner along the near side.
Place the rubber sheet 18 inches from the head part of the mattress. Place the draw sheet.
Spread top sheet on the mattress with wrong side up, making even the edges of the bed. Draw all linens tight and tuck under the mattress.
Fold top sheet back to 18 inches
Place the pillow case and put it at the head part of the bed. Cover with top sheet.
Fold the towel into three sections and fold into two.
Hang over at the top of the bed with free or loose ends out of sight

OPEN BED

Prepare the materials needed (same as closed bed)
Replace the top sheet with bath blanket. Loosen all the bed linens from all sides of bed. Go to opposite side of bed and draw the patient towards you. Return to the work side of the bed.
Fanfold the bottom sheet, rubber and draw sheets in irregular folds to the center of the bed.
Place a clean bottom sheet, the rubber sheet and the draw sheet on the bed in the position described for closed bed.
Tuck the top sheet, miter the corner then pull the loose ends on the bed and the draw sheet. Tuck in together with the bottom sheet.
Fanfold the halt of the clean sheet close to the patient along with the half of the rubber sheet and the draw sheet.
Roll the patient gently over the folded sheets to the near side of the bed.
Go around to the other side of the bed, remove the soiled bottom sheet and pull the fan folded sheet at the side and make bed as it was done on the opposite side. Fold the soiled linens and place it at the foot part of the bed.
Remove the bath blanket and put on the top sheet. Miter the corner as in the closed bed but care is taken that they are loose enough at the foot top to prevent pressure on the patient's feet.
Straighten the upper edge of the cover, fold neatly and put clean pillow
Change the soiled wash cloth, bath and hand towels with fresh ones.
Rerturn chair and bedside table to proper place
Place all soiled linens in the linen hamper.

Bed making is just the most tiring (I think) return demonstration we had we had. Though I'm good in making my own bed, this one's entirely different! I hate the folds in the blanket and you have to get it right to spread correctly the sheets and to get that HIGH SCORE! GRRR! I don't care with my grades either, as long as I can perform. ^_^ I'll see if I can post about techniques in bed making in my next post! ^_^
